Based on your answers, we may request medical records directly from your veterinarian to confirm any pre-existing conditions. No action is required on your part at this time. If pre-existing condition exclusions are placed on your policy, you will receive an updated Declarations Page in the mail.
A pre-existing condition is any illness or injury that your pet had before coverage started. This includes any condition for which your pet displayed signs or symptoms, was diagnosed or was treated prior to your policy effective date or during your waiting period. The good news is, not all pre-existing conditions are excluded permanently. If a pre-existing condition exclusion is placed on your pet's policy, you may be able to get it removed by requesting a review. It's also important to remember that, even though pre-existing conditions aren't eligible for coverage, your policy covers of hundreds of other conditions everything from lacerations to lymphoma.
If you have medical records from your vet showing that your pet's pre-existing condition is not chronic and has been cured for a minimum of six months, you may be able to have it removed. Review forms and instructions on how to request a review can be found at www.petinsurance.com/forms
You are not required to complete the questionnaire for enrollment. However, this may extend the claims process if we determine that medical records are needed.
